Ohio State fired wide receivers coach Zach Smith last summer over criminal charges and allegations he had abused his wife.

Ohio State formed an independent investigative committee to see if former head coach Urban Meyer and staff covered up Smith's behavior before his firing.

Meyer and athletic director Gene Smith were both suspended after the committee determined several missteps in handling of Zach Smith's behavior.

Meyer retired as football coach in January.

An Ohio State University investigation in 2014 found then marching band director Jonathan Waters allowed a "sexualized culture" in the band. He was subsequently fired.

The investigation, which was done internally by Ohio State, found, among other things that some band members carried on traditions of harassment of women that sparked complaints from parents.

Ohio State fired its head cheerleading coach in 2013 after she failed to report the sexual misconduct of two assistant coaches.

Lenee Buchman was fired by athletic director Gene Smith after cheerleaders complained of being harassed.

An investigation found Buchman had removed a cheerleader from the team after he complained about an assistant coach's behavior.
